This form will allow you to report an incident of hazing to the Office of the Dean of Undergraduate Students.  By filling out this form, we may be able to determine your identity.  The Dean's Office encourages reporting parties to identify themselves as it is more helpful in conducting interviews and clarifying details.  While we will do our best to honor a reporting parties' privacy, we cannot guarantee it.  Individuals who wish to make a report but prefer to remain anonymous should do so using EthicsPoint.
